Appointments

Please bring the following items to your appointment:

Completed Application, form DS-11, available at travel.state.gov
Proof of U.S. citizenship – original or certified copy of his or her U.S. Birth Certificate, Certificate of Birth Abroad, or Certificate of Naturalization
Photo Identification – Driver’s License, Government or State-issued ID card
Application Fee – Payable to U.S. Department of State by CHECK or MONEY ORDER only.
Amounts vary depending on product & service requested. See fee schedule.
Execution Fee ($35.00) – Payable to Indian River State College.
Passports for Minors (Under 16 years of age) Both parents, along with the child, must be present with photo identification (Driver’s License, Government or State-Issued ID) See the parental consent section for additional information.

Already Have A Passport? – If you already have a passport, you may be eligible to renew it by mail.
